Family Connects NJ Case Managers
The Cooperative’s Family Connects NJ program is hiring two full-time Case Managers to serve families throughout South Jersey, with a particular need for candidates to serve Atlantic, Cape May, Cumberland and Salem Counties.
Case Managers coordinate safe, timely, effective, equitable and client-centered care, linking families to community resources and providing ongoing support, advocacy and follow-up. As key members of the Family Connects NJ interdisciplinary team, they attend team meetings, participate in case conferences and document in HealthCloud.
Essential Functions:
- Effectively cross-collaborate with agency staff, leaders and board members to foster a comprehensive approach of information sharing, working committees and collective representation
- Routinely disseminate relevant information, presentations, training and learning opportunities for internal program and agency staff
- Ensure timely response when following up with families, new enrollments, work with families, prompt documentation in HealthCloud and community stakeholders to ensure viable resource options for families in general
- Use critical thinking and problem solving to determine the best care plan for each client after assessing clients
- Ability to organize, prioritize and manage a diverse caseload while meeting established timelines and quality standards
- Assist in identifying gaps and barriers to services and system issue trends families experience in utilizing services
- Refer clients to and support the facilitation of existing postpartum support groups in the South Jersey region
- Advocate on behalf of the families
